Person Centred Planning

 

A person centred approach is the foundation of our work. We believe that every person has the internal resources to really flourish and be who they can be. This approach supports good relationships and it influences every aspect of our work, from recruitment and finance management through to the way employees of the organisation and the people we work for are supported.

Person Centred Planning grew out of these beliefs. It is the way that we ensure that a person's needs, desires and wishes guide the support they receive and provide an overall direction for their life. Everyone has the opportunity for a Planning Day at least once a year and a follow up session no more than six months later to ensure things are on track.

Planning Days encourage all the people who know and care about a person to come together to help share ideas about what they would like to do in the year ahead. A Planning Day can be fun, serious and informative and takes place wherever the person wants. Some people choose to have a planning day with a theme.

A graphic record is kept of the day so that everyone remembers what plans were made and what they agreed to do to make sure the plan happens.
