Supported Living

We believe that the support you receive should be provided in the way that you want and by people you have chosen to have work with you.

We all have our personal preferences in terms of the kind of people we like and want to spend time with.  It is important these preferences are respected when considering who will support you with aspects of your daily living.  Your support should;

• Be provided by your specific, locally recruited team

• Help you recognise all your social, family and community assets

• Be designed in a way that makes sense to you

• Be flexible and at a time that makes sense to you (no fixed shifts)

• Provide support in your home if you want it

• Support you out with your home to access education and other community activities

• Include person specific training available to team members and the family

• Include the opportunity to take a short break or holidays if you need this.

• Have transparent individualised funding arrangements

• Allow access to iPLANIT outcome based support plan software (web based).
